Root Cause Analysis
This tool can be used to help identify issues that may have contributed to, or directly caused an incident.

In this method, six areas are considered for investigation:

1. Culture – Policies – Systems of Work
2. Information
3. Work Environment
4. Plant – Equipment – Tools
5. Materials – Substances
6. Persons
